Hi all
Temora High school is involved in MindMatters, a mental health initiative for secondary schools, and part of the initial process is to gather information about the perceptions of the school community regarding mental health and wellbeing. Your responses to this survey will help inform the school's future work on mental health. We have chosen MindMatters because it is an ongoing whole school and community approach to educate and create an awareness of mental health, general wellbeing being and strategies to improve these.
This survey contains 28 questions and will take about 10 minutes to complete. Your responses will remain anonymous and confidential.

What is MindMatters?
MindMatters is a mental health initiative for secondary schools that aims to improve the mental health and wellbeing of young people. It recognises the important role that parents and carers play in the lives of their children and encourages effective working relationships between school staff, parents and carers and students as well as the broader community.
What choice do I have?
Completing this anonymous survey is entirely your choice and you can choose not to answer some or all of the items. Whether or not your decide to participate, your decision will not disadvantage you or your child. However, we hope that you will contribute to helping improve your school community. Please feel free to discuss any concerns you may have about completing this survey with any of the staff.
How is my privacy protected and what happens to the answers?
Your answers to this survey are completely anonymous. Data is stored online by beyondblue as part of MindMatters in a password secured database, in accordance with the Australian Privacy Act 1988. The school cannot access the raw data. Only summarised results are available to the school to assist with school planning. MindMatters may also use de-identified data summarised and reported at a national level in ways that inform the national development of the MindMatters initiative. Paper surveys will be entered into the online database by the THS MindMatters team.
What do I need to do to participate?
Please carefully read the instructions and information statements. If there is anything you do not understand, or if you have questions, please talk to a school staff member.
If you consent to participate, please complete the survey according to the instructions. If you are using the paper survey please return to the front office. Both forms of the survey should be completed before Friday 20th November.
